Adolescent Transitions

General Info

Transitions programs are designed to help adolescents with disabilities, ages 15-21, move into adulthood and develop skills for future success. The program focuses on supporting individuals with emotional and behavioral challenges with skill developments.

Professional Info

Easter Seals UCP continues to explore avenues to expand programs for youth transitioning to adulthood.  Currently, the Transition Program based in New Bern is designed to assist and empower young people with mental health and/or substance abuse challenges in making a successful transition to adulthood.  Additional program models and locations are being explored for future implementation.
